The Vitamin D Deficiency Almost Nobody Tests For

There is a number that describes your vitamin D status, and almost certainly you do not know it. It is called 25-hydroxyvitamin D, measured in nanograms per milliliter, and it is not on the standard blood panel your doctor orders at a check-up. You get a lipid panel, a metabolic panel, maybe a thyroid test if you ask. Vitamin D is an add-on. Someone has to think to order it.

That is the whole problem in one sentence. This is one of the most common nutrient shortfalls in the developed world, its symptoms are quiet and easy to blame on something else, and the one test that would catch it is the one nobody runs by default. Roughly 40 percent of American adults come back below the deficiency threshold when they are actually measured. Forrest and Stuhldreher, analysing NHANES data across more than 4,000 adults, put the figure at 41.6 percent under 20 ng/mL (Nutrition Research, 2011). Not a fringe group. Close to half.

I want to walk through what that number actually means, why the lab's idea of "normal" is a floor rather than a target, and what the honest version of the evidence says once you strip out both the supplement-industry hype and the reflexive backlash against it.

Deficient and optimal are two different questions

When a vitamin D result does get run, the report usually flags anything under 20 ng/mL as deficient and calls everything above it normal. That cutoff exists for a specific reason: it is roughly the level below which bone problems like rickets and osteomalacia start to appear. It is a floor for skeletal disease, not a description of where your body works best.

The gap between those two ideas is where most people live. A result of 22 ng/mL clears the deficiency flag and prints as normal, so you never hear about it. But that is barely above the line, and many researchers who study vitamin D argue the range associated with the lowest all-cause mortality sits higher, somewhere around 40 to 60 ng/mL. Whether the true optimum is 40 or 30 is genuinely debated. What is not debated is that "not deficient" and "optimal" are answers to two different questions, and your lab report only answers the first one.

This is the same trap that shows up across nutrition. An RDA is the amount that prevents a deficiency disease in most of the population, which is a different thing from the amount that supports the best long-term outcome. The reference range is built around preventing a bone disease from the 1900s, and then quietly reused as if it described thriving.

The symptoms are real, and that is exactly why they get missed

Here is the frustrating part. Low vitamin D does have symptoms. Fatigue that sleep does not fix. Low mood, especially in the darker months. Frequent minor infections. A vague ache in the bones or muscles, sometimes in the lower back or hips. Muscle weakness that shows up as struggling on stairs you used to take easily.

The trouble is that not one of those is specific. Fatigue is a symptom of almost everything. Low winter mood could be a dozen things. Getting sick more often gets blamed on stress or bad luck. Every one of these signs points at vitamin D and at fifty other explanations at the same time, so it rarely gets pinned on the actual cause. You adjust to feeling slightly worse and call it a busy season of life.

This is what I find genuinely difficult about the whole nutrient. A dramatic deficiency announces itself. A chronic, moderate one just files the edges off how you feel and hides inside a normal-looking life. It is the same pattern I keep running into with the deficiencies that never show up on a standard check-up: you feel roughly fine, your bloodwork looks fine, and something is quietly running at reduced capacity for years. With vitamin D the difference is that a single test would settle it, and hardly anyone gets the test.

Sun, food, supplement, in rising order of difficulty

Your body is built to make vitamin D from sunlight. Ultraviolet B hits the skin, converts a cholesterol derivative, and the liver and kidneys finish the job. In principle you need no food source at all. In practice the sun route fails for most people for reasons that have nothing to do with willpower.

Latitude is the big one. Above roughly the 37th parallel, which includes most of Europe, Canada, and the northern United States, the winter sun sits too low to deliver meaningful UVB for months at a time. You can stand outside at noon in January and make almost none. Add the rest of modern life on top: indoor jobs, glass that blocks UVB, sunscreen that blocks it by design, and darker skin, which is more protected against UV and therefore synthesises vitamin D more slowly. Every one of those is a good and normal thing. Together they mean the default human vitamin D source is switched off for a large share of the population much of the year.

Food helps but cannot easily carry the load, because very few foods contain much. Fatty fish is the real source. A three-ounce serving of wild salmon gives around 570 IU, sardines about 164 IU. A tablespoon of cod liver oil is an outlier at roughly 1,360 IU, which is why it was the old-fashioned remedy. After that it drops off fast: egg yolks bring about 44 IU each, and UV-exposed mushrooms are one of the only plant sources worth naming. Against a target that many researchers put in the low thousands of IU per day, a normal diet without oily fish several times a week simply does not get there.

Which leaves supplementation, and here vitamin D is unusual: it is cheap, well absorbed as D3, and easy to get right. But easy to get right is not the same as pointless to measure. The supplement makes sense precisely because you have confirmed a low number and cannot fix it with latitude or salmon alone, not as a reflex you take because the internet told you to.

The honest version of the evidence

This is where I have to be careful, because vitamin D is a topic where both the boosters and the sceptics overstate their case. Let me give you the version I actually believe.

For years, observational studies kept finding that people with low vitamin D had more of nearly everything bad: heart disease, cancer, infections, depression, earlier death. That is real, and it is why the supplement enthusiasm took off. But a large systematic review by Autier and colleagues in The Lancet Diabetes & Endocrinology made an uncomfortable point: much of that association may run backwards. Illness and inflammation drive vitamin D levels down, so a low reading is often a marker of poor health rather than its cause. If that is true, then dosing a sick person with vitamin D would not undo the underlying disease, and the observational studies would have oversold the pill.

The large randomised trials mostly bear that caution out. VITAL, run by Manson and colleagues and published in The New England Journal of Medicine in 2019, randomised nearly 26,000 adults to 2,000 IU of vitamin D3 a day or placebo and found no significant reduction in major cardiovascular events or total cancer over about five years. That is a serious result and it should temper anyone selling vitamin D as a longevity drug. The catch, and it matters, is that VITAL enrolled a general population that was mostly already vitamin D replete. It was not a test of correcting deficiency. It was a test of adding more to people who largely did not need it.

And on the outcome vitamin D is most plausibly tied to, the picture is a little warmer. An earlier meta-analysis of supplementation trials by Autier and Gandini in Archives of Internal Medicine found a modest reduction in all-cause mortality, a summary relative risk around 0.93. Modest, not miraculous. Put it together and my read is this: vitamin D is not a wonder supplement that adds years to a healthy person who is already at 45 ng/mL. But correcting a genuine deficiency is a real and worthwhile thing, and the only way to know which situation you are in is to measure. Vitamin D does not work alone

One reason the megadose approach can backfire is that vitamin D is a node in a network, not a standalone lever. Its main job is to raise calcium absorption from the gut. That is helpful when the calcium then goes where you want it. It is not helpful if the calcium ends up in your arteries instead of your bones, and vitamin D by itself does not decide where it lands.

That decision belongs largely to vitamin K2, which activates the proteins that pull calcium into bone and keep it out of soft tissue. Maresz laid out this cooperative relationship in Integrative Medicine: push calcium absorption up with vitamin D but leave K2 low, and you can end up routing that extra calcium into places you do not want it. Most Western diets are low in K2, which comes from natto, aged cheeses, egg yolks, and organ meat, so the cofactor is often the weak link.

Magnesium is the other quiet partner. The enzymes that convert vitamin D into its active form depend on it, so someone low in magnesium can supplement vitamin D diligently and still not raise their active levels the way they expected. Given that nearly half of adults fall short on magnesium in the first place, this is not a rare edge case. The takeaway is simple: vitamin D is worth getting right, but not in isolation. A big dose of D on a foundation of low K2 and low magnesium is not the win it looks like on paper.

What I actually do with this

My own approach is boring on purpose. Know the number, feed the system, then decide about a supplement rather than the other way around.

Knowing the number means asking specifically for a 25-hydroxyvitamin D test, because it will not be there unless you ask. Feeding the system means eating oily fish a couple of times a week for vitamin D, keeping a real K2 source in the rotation, and not neglecting magnesium, so that when D goes up the rest of the machinery can actually use it. Getting sun when the season and latitude allow it is free and does more than any single food. Only after all of that does a supplement become a considered decision aimed at a measured gap, usually a modest daily D3 through the winter, rather than a shrug.
